Mercy Heart Failure Center - Joplin is a support program for patients diagnosed with heart failure (HF). We help HF patients stay in control of their health by teaching them about HF symptoms and treatment, and by helping them to manage the disease process.

Our program includes:

  • Telemonitoring of patient symptoms – We telephone patients on a regular basis. Specially trained cardiac nurses monitor symptoms over the phone and can often intervene early if symptoms worsen.
  • Patient education - Patients receive ongoing education about HF, its early warning signs, dietary guidelines, medication management and lifestyle issues.
  • Nutrition consultations - Patients may consult with a dietician if needed to help them adopt healthy eating habits.
  • Medication counseling - Patients learn about their medications and proper use.
  • Heart Failure rehabilitation exercise program – Stable patients may participate in this monitored exercise program developed specifically for HF.
  • Monthly Heart Failure group - This special gathering provides emotional support for patients and family members.
  • Referrals to other appropriate community resources - We connect patients with needed services and education.
